Initial Enrollment for Supplemental Life/AD&D Insurance

When you become eligible for Infillion benefits as a full-time employee, you have a one-time opportunity to enroll in Supplemental Life/AD&D Insurance for you and/or your spouse without Evidence of Insurability (EOI) up to certain limits ($600,000 for employees and $50,000 for spouses). Employees who previously declined supplemental life/AD&D insurance for themselves or their spouse will be subject to EOI for any amount elected.

What is Evidence of Insurability?

Evidence of Insurability (EOI) is an application process in which you provide information on the condition of your health or your dependent's health in order to be considered for certain types of insurance coverage.

If you are required to complete EOI, you will receive an email from Prudential to register online and complete the EOI application. If you do not have an email on file, you will receive a paper application mailed to your home address to complete and return to Prudential.

If it is not submitted timely, the benefit amount elected could be denied.
